We are looking for an in-home Personal Care Attendant to add to our team of professionals. We are looking for experienced team members who have a high touch, skilled approach to providing care. Our Personal Care Attendants will not only provide assistance with activities of daily living, but also assist with an exciting and stimulating activity program tailored to each client. At Sage Eldercare Solutions, we work as a team, and these individuals work closely with the Home Care Director to provide a very unique and detailed activity plan that makes the day meaningful and exciting for both the client and the Personal Care Attendant. The Personal Care Attendant will play a crucial role in the success of the day to day experience of the client. We believe this is a unique and special opportunity unlike anything else you will find in the industry.
Qualifications
 Completion of a CNA or CNHA Certification and/or 2 years aide experience.
 Ability to read and follow instructions; communicates clearly; writes legibly.
 Self-directing with the ability to work independently.
 Patience and empathy for the ill, injured, frail and those with disabilities.
 Flexible and cooperative in fulfilling role obligations.
 Practices good personal hygiene.
 Demonstrates good observational skills and sound judgment.
 Experience with and/ or demonstrated knowledge of Plans of Care.
 Has satisfactory report of pre-employment physical.
 Ability to carry out activity planning while maintaining an upbeat, positive and creative atmosphere.
 Complies with legal documentation for work.
 A minimum of three references from existing or prior care attendant employers.

Summary of Job Responsibilities
The Personal Care Attendant will work under a supervised plan of care or with appropriate supervision to assist the client and family in providing personal care services.
Essential Duties
Responsibilities of the Personal Care Attendant include, but are not limited to, the following and are provided in accordance with the Plan of Care:
• Provides housekeeping, home management, and laundry to maintain a safe environment.
• Assists and/or provides all aspects of personal care including bathing, dressing, oral hygiene, hair care, and shaving.
• Encourages client to maintain independence.
• Assists and supports clients with ambulation and transfers as needed.
• Helps with specialized assistive devices; e.g. however lift, board, etc.
• Assists with prescribed exercises and rehabilitation.
• Assists with toileting as needed.
• Prepares meals, planning and shopping for regular and specialized diets.
• Assists with eating.
• Observes and reports intake of fluids and food and frequency of elimination if indicated.
• Assists with medications that are self-administered.
• Works closely with Home Care Director to create meaningful activities.
• Closely follows care plan provided by Home Care Director and conducts activity program on a daily basis.
• Communicate with Home Care Director when art or personal care supplies are needed.
• Observes and monitors the client's condition, notifying Care Management staff of any significant change in client's status.
• Provide documentation as needed (daily log entries, time sheets, etc) and attend necessary training (paid).
• Complies with Sage Eldercare's Policies and Procedures

Working Conditions
1. In-home (client residence) environment.
2. Possible exposure to infectious diseases.
3. In and out of automobile or public transportation, if applicable.

Continuing Education Requirements
Personal Care Attendant is expected to participate in appropriate paid trainings as requested and/ or required by Sage Eldercare.
