Q.6...... An AIDS patient:
A 31-year-old man with AIDS is receiving highly active antiretroviral therapy. Labs show significant myelosuppression and a decreased plasma HIV-mRNA burden. To counteract this toxicity, he begins treatment with a hematopeoietic growth factor. Thirty minutes later, he develops dyspnea, severe muscle pain, vomiting, and sinus tachycardia; his BP is 80/40 mm Hg. Which of the following hematopoietic GFs most likely caused these new finding:
A. Fligrastim(G-CSF)
B. Sargramostim(GM-CSF).
You answered Fligrastim,
But I think the answer is Sargramostim, since they side effects are similar, but Sargramostim's side effects are more severe, like in this patient.

Q.6.....

-Among the two drugs Sargramostim is well known to have the so called First Dose effect... the side effect happened on the first dose and 30 minutes later ....

• A “first dose effect” may occur, which is characterized by an increased heart rate, feeling faint, low blood pressure and reddening of the face. This tends to only happen on the first dose of sargramostim
• Diarrhea or stomach upset
• Swelling, pain, redness, tenderness at the site of injection
• Flu-like symptoms that may include fever, headache, achiness or pain, weakness and fatigue
• Bone pain
• Swelling of feet or hands

  @lovetest and Maryam...I compeltely agree with ur answer of que 6 ie D sagramostin...since it seems like the first dose effect seen with sargrmostin as explained by maryam...thnk u guys :)
