 I typically will consider doing a Revision Open Rhinoplasty with Ear cartilage to the tip to bring down the tip at 6 months.  You may also be describing what&#039;s called &quot;alar retraction or alar notching&quot;.  I correct this with a small triangular piece of ear cartilage with skin attached which when properly placed...acts to lower the edge of the nostrils.  Of course, without seeing pictures, I can&#039;t be certain which...or both...of these conditions exist in your particular case.  Now, if you indeed are a candidate for either of these procedures as a Revision Rhinoplasty it would be a minimum of 6 months from the last Rhinoplasty and would depend on the condition of your nasal skin and my determination (or any experienced plastic and cosmetic surgeon with Revision Rhinoplasty expertise) whether or not your tissues could undergo another Rhinoplasty.  The correction of alar notching or alar retraction is a much less invasive procedure and would typically be able to be performed at 6 months post-op.
