Miriam,

I've been performing Rhinoplasty and Revision Rhinoplasty for over 20 years and over that entire time, I believe it's been common knowledge among Rhinoplasty Surgeons that patients with thick, oily skin are at increased risk of developing additional internal scarring after a Rhinoplasty and this excess scar most commonly occurs in the supra-tip region. If it's severe, the term "polly beak" deformity is used to describe the condition.

I obviously can't address your question regarding why this wasn't discussed by the plastic and cosmetic surgeon that did your Rhinoplasty but I've preached and preached on this blog till I'm purple in the face about having the most experienced Rhinoplasty surgeon you can afford. This is one of the reasons...the time and knowledge to have that discussion with you pre-op.

Kenalog 10 is my preferred treatment to control excess scar formation along with specific daily masssage of the nose. Fibrin glue has no bearing , that I am aware in causing or preventing increased supra-tip scar formation in patients with thick skin.

Laser re-surfacing of the nose is a way to decrease the size of the nose by thinning the skin from the outside...done typically 1 year after the Rhinoplasty but will not decrease the amount of internal scarring.

Revision Rhinoplsty where the scar tissue is removed is the only way to remove the scar tissue that renmains after the nose is stable from the Rhinoplasty Surgery.

Hi Dr. Palmer,

I also meant to ask about the use of Fibrin Glue for prevention of Supratip deformities during first rhinoplasties and revision rhinoplasties. Have you heard of this or know if it works?
Thank,
Miriam

Hi Dr. Palmer,

I am a middle eastern 26 year old woman with clearly oily skin. I had rhinoplasty for a dorsal hump 1 year and 2 months ago. 2 months post op, I began to develop a supratip deformity, which my doctor is attributing to my skin type. However, he never discussed the possibility of a supratip development with me before the surgery, even though it was clear that I have very oily, thick skin, especially on my nose. Hence, I was never told that there were methods to help prevent this problem, including Kenalog and supratip taping. My surgeon has been using Triamcinolone injections since 4 months post operatively, and I just had my 5th injection. Is the laser skin resurfacing a good option for me? Are there other medical options that I have before we resort to a possible revision rhinoplasty, since I still have some supratip. Also, is it common practice not to warn a patient about what is now apparently a common complication that could have been predicted given my skin type?

Patrick,

It's my understanding that the kenalog starts working immediately and continues to do so for some several months.

Dr. Palmer,
My PS gave me kenalog shots 3 weeks post-op after my rhinoplasty. He said it will take 2 weeks for the shots to take effect. Is this true or did I misunderstand what he said? And why does it take 2 weeks to "kick in"?

Thanks
